For people living with disability, transitioning to a new stage of life can be a challenge. That’s why MKM Disability & Care is here to assist participants through such demanding moments. There are many life-stage transitions we can help with – including starting or changing school, moving home or into supported accommodation and even getting involved in the community through events and activities.
We are here to help when you need us
We support participants planning for change, including developing strategies to cope with the stress they may experience during transitions and ensure that it positively impacts their overall wellbeing. Our approach is customised for each participant, considering their abilities and needs.
The assistance we provide includes preparing schedules (daily, weekly and/or longer-term). This is to help ensure that the support being provided assists a participant’s life stage transition goals. We can also help with the budgeting process to plan and pay for expenses when due.
