How we can support you making the most of your NDIS plan
Whether it’s the first time you are accessing the NDIS or have been a Participant in the scheme for a while, we will ensure that you fully understand and make the most of your NDIS plan, connecting you to the services you need.
Our NDIS Support Coordination team will work together with you to define your goals and needs and provide advice on the best solutions to help you meet them. As Support Coordinators, we work with you to:
01. Understand your goals and identify your needs
We will gather information about your lifestyle, health, requirements and preferences to understand your objectives and know how we can help you.
02. Understand and maximise your NDIS plan
We will assist you in making the best choices for you and coordinating your care plan. We will work with you throughout every step of the process to get the most from your NDIS plan.
03. Connect you to NDIS providers
We will connect you with the right service providers, community, and other government services. We ensure service arrangements are undertaken as scheduled and you or your loved ones start receiving the care you deserve. MKM Disability & Care works only with certified NDIS providers.
04. Ongoing advise and guide
MKM Disability and Care will provide advice and support throughout your NDIS journey. We’ll help you We’ll help you analyse the progress of your goals and needs, and address any provider issues that may arise. Also, we assist you in creating relationships, removing barriers in the support environment and making the NDIS process easier for you and your loved ones.
